Before you can benefit from CIGNA Dental Implant Extra cover your dentist needs to be registered with CIGNA.

  • Download the patient application form and print off
  • Fill in patient application form
  • Download the dentist application form and print off
  • Ask your dentist to complete the dentist application form
  • Return both forms to CIGNA

Your price is based on the cost of your dental implant treatment. Payment is by monthly direct debit.

If you're a dentist

Your patients are only eligible for CIGNA Dental Implant Extra if you are registered with us. So we ask you to complete a short questionnaire to confirm implant experience and activity.

Simply download the questionnaire then complete and return to CIGNA.

Once you're registered with CIGNA then you’re ready to become an introducer.
